Erfolgreiche Teilnahme für aycan und OsiriX PRO beim Workstation Face Off in Garmisch-Partenkirchen

Beim diesjährigen Face Off während des ausgebuchten 7. internationalen Symposium Mehrschicht CT (CT2012) traten vier Firmen (GE, Siemens, Vital Images und aycan) an. Von den Zuschauern wurden weitere Firmen vermisst (Carestream, Philips, Visage Imaging, Terarecon), die sonst auch immer bei diesen "Wettkämpfen" teilnehmen.

PD Dr. A. Graser und Prof. H.-C. Becker vom Institut für Klinische Radiologie des Klinikums der Universität München moderierten gewohnt souverän die 4. Face Off Session in Garmisch-Partenkirchen die beiden Fälle.

Der erste Fall war eine CTA eines Aortenstent Patienten mit Endoleak und einem Bypass. In 5 Minuten mussten eine Volumetrie des Aneurysmasacks in zwei Zeitpunkten erstellt werden. Die dynamische Multiphasen CT Serie mit 5404 Bildern für die Darstellung des Endoleaks stellte die nächste Herausforderung dar. Am Ende wurde der Bypass in der curved MPR und im Volumerendering gezeigt.

Der zweite Fall legte die Messlatte deutlich höher. Der onkologische Fall eines Patienten mit multiplen Lymphomen forderte alles von den Systemen und Präsentatoren. Neben der Volumetrie eines Lymphoms in zwei Zeitpunkten und dem Vergleich des SUVmax Wertes wurde ein Lymphom-Staging nach Cheson in drei Zeitpunkten gezeigt. Der Umgang mit drei PET-CT Untersuchungen (Synchronisierung, Registrierung) stellte den Abschluss der 6 Minuten Präsentation dar.

aycan zeigte eine Spitzenleistung auf Augenhöhe mit den anderen Anbietern.

Immer wieder hören wir, dass PACS Kunden mit ihrem – meist großem – PACS-Anbieter unzufrieden geworden sind. Frohe Weihnachten vom aycan Team

Zu diesem Zweck haben wir die Fenster im 1. OG unseres Bürogebäudes in Würzburg mit bunten Klebezetteln verschönert. Die Klebezettel sind Pixel/Voxel, mit denen wir in unserem Berufsalltag ständig zu tun haben.

Dieser sogenannte Postit-War fand seinen Anfang im Sommer 2011 in Paris.

Frederick Memorial Health Care System installs additional aycan xray-print solutions

DICOM paper-print solution saves film costs and improves communication with referring physicians and patients

We have successfully installed two additional aycan xray-print solutions at Frederick Memorials’ health care facilities in Frederick, Maryland. Since 2007, aycan has installed a total of six systems for the health care system.

Frederick Memorial chose aycan xray-print, a vendor neutral DICOM paper-print solution, to replace film printers in all of its six health care facilities to help improve communication with referring physicians and patients and reduce costs. Compatible with PACS and modalities using the DICOM 3.0 standard, the aycan xray-print solutions fit seamlessly into Frederick Memorials’ existing workflow.

“In addition to the tremendous cost savings, aycan’s expert knowledge of DICOM, the very smooth integration, and excellent continued service and support are the reasons we chose them again to replace our remaining film systems,” said Brian Petrie, PACS Administrator at Frederick Memorial. “Since our first aycan xray-print installation in 2007, we have achieved well over 70% savings in our annual film costs as well as improving communications with our customers.”

aycan xray-print prints images on plain paper in color or black and white at less than 10 cents per sheet. Customers replacing film with paper also achieve additional savings through the elimination of expenses associated with the special handling, storage, and transportation of film.

Today, diagnosis, based on follow-up image series, is often a time-consuming task due to missing data alignment and the need for manual adaptions of viewing settings. Im Diakonissenkrankenhaus setzt Chefarzt Professor Müller-Hülsbeck die Lösung seit 4 Wochen das iPad für die Visite ein.

Risiko mit medizinischer Software - ein Beispiel aus dem echten Leben

Vor kurzem unterhielt ich mich mit dem Vertriebsleiter, der hier nicht namentlich genannt werden möchte, einer internationalen Medizintechnikfirma über das Thema "Risiko von medizinischer Software".

Er berichtete von einem Fall, bei dem zwei Patienten gleichen Geschlechts, ähnlicher Körpergröße und Gewicht von der PACS Software vertauscht wurden. Beide Patienten hatten auch die gleiche Erkrankung an der Niere, nur auf unterschiedlichen Seiten. In der Folge wurde ein Patient operiert und im wurde seine gesunde Niere entfernt.

Der ganze Fall landete vor dem Richter und alle Beteiligten (Ärzte, Klinikangestellte, Hersteller der beteiligten Medizinprodukte) in der Diagnose- und Therapiekette wurden geladen. Auch der Vertriebsleiter selbst war geladen und musste viele Fragen über sich ergehen lassen. Die Juristen legten größten Wert auf Protokolle, Zulassungen und Zertifikate der beteiligten Personen, Firmen und Medizinprodukte. Diese Erfahrung war sehr lehrreich für den Vertriebsleiter, er möchte sie aber nicht wiederholen.

Warum berichte ich hier darüber?

Weil es immer wieder erstaunlich ist, wieviele Ärzte Software in der Diagnostik und Therapie einsetzen, die nicht über die notwendigen Zulassungen verfügen. Das Argument "Ich schaue ja nur mal...." wird gerne als Schutzbehauptung angebracht. Dabei gibt es mehr und mehr auch zertifizierte Alternativen beliebter Open Source Software.

Mich würde interessieren, wie die Ärzte in der oben geschilderten Situation reagieren würden, wenn eine Software ohne Zertifizierung als Medizinprodukt für die Vertauschung verantwortlich gewesen wäre.

Das iPad polarisiert die Experten in deutschen Krankenhäusern

Mittlerweile haben wir mit unserem xr.ay App für die Teleradiologie, Visite und Zuweisermarketing bereits einige Präsentationen in deutschen Kliniken gemacht. Die Reaktionen sind sehr unterschiedlich und zeigen, wie das Thema "Mobile Health" polarisiert.

Man braucht nur ein iPad einer Runde von Klinikern, Verwaltern und EDV'lern zu zeigen, schon kochen die Emotionen hoch. Das iPad ist ein regelrechter Katalysator für kontroverse Diskussionen.

Während eine Gruppe (meist die Ärzte) die Technologie als Weg in die Zukunft sehen, wird sie von anderen Präsentationsteilnehmern als indiskutabel bezeichnet.

Die Befürworter sehen in erster Linie die Freiheit und die Schnelligkeit durch den direkten Zugriff auf Informationen (DICOM-Bilder, RIS/KIS Daten, Lehrbücher, Fallsammlungen, etc...) auf Station oder unterwegs als großen Vorteil.

Kritiker haben Bedenken in punkto Sicherheit, was ein wichtiger Aspekt bei einem extrem mobilen Gerät, wie dem iPad richtig ist. Der Zugang zu medizinischen, personenbezogenen Daten erfordert ein hohes Mass an Sicherheit. Der Verlust von iPads innerhalb oder ausserhalb der Klinik stellt ein Risiko dar. Der strenge, deutsche Datenschutz spielt dabei auch eine Rolle.

Ein Blick in die USA zeigt, dass der mobile Zugriff auf medizinische Daten nicht mehr aufzuhalten ist. Nun ist es an den Herstellern und Anwendern diese Technologie verantwortlich zu nutzen.

Der richtige UMTS Tarif für das iPad in der Teleradiologie

Wenn man seine DICOM Bilder per UMTS auf das iPad xr.ay App schicken möchte, sollte man den richtigen Tarif wählen. Das iPad benötigt eine MicroSIM, diese ist etwas kleiner als die herkömmlichen SIM Karten.

Es gibt im Prinzip vier große Anbieter, die das Netz zur Verfügung stellen (Telekom, Vodafone, O2, e-plus). Aufbauend auf diese Netzanbetreiber gibt es mehr als 30 Anbieter von Datentarifen.

Hier sollen einige Aspekte beleuchtet werden, die als Grundlage für die richtige Auswahl dienen können.

1. Netzabdeckung
Die Netzabdeckung der 4 großen Provider läßt sich online recherchieren. Nicht jeder Provider bietet in jedem Gebiet eine UMTS Abdeckung. Vor allem in ländlichen Gebieten muss man mit langsameren EDGE/GSM Netzen rechnen.

- Vodafone Netzabdeckung Vodafone UMTS
- Telekom UMTS/HSDPA, GSM/GPRS/EDGE und WLAN Netzabdeckung
- O2 UMTS / HSDPA / Edge - Netzabdeckung
- E-Plus UMTS - Netzabdeckung

2. Geschwindigkeit
Die Übertragungsgeschwindigkeit hängt vom Netzstandard und der Anzahl der eingeloggten User in einer Funkzelle ab. Momentan lassen sich Download/Uploadgeschwindigkeiten von max. 21/5,8 MBit/s erreichen.
Tipp: Beim Übertragen ist zu beachten, dass der Uploadkanal aus der Praxis oder Klinik bei ADSL durchaus eine viel geringer Bandbreite liefert, als der Downloadkanal bei UMTS. Das ist dann das Nadelöhr für den Transfer.

3. Kosten

Eine volumenbasierte Abrechnung kann teuer werden, da pro MB 0,24 - 0,35 € verlangt werden. Ein Schädel CT mit 50 Bildern kann dann 3,50 € Transferkosten verursachen.

Für den Radiologen empfiehlt sich eine sogenannte Flatrate.
Die meisten Anbieter verlangen eine Bereitstellungsgebühr und dann eine Gebühr für das Datenvolumen. Dabei wird ein bestimmtes Datenvolumen mit Zeitbegrenzung (Monat, Tag, Stunde) für einen pauschalen Preis zur Verfügung gestellt. Tagespässe werden bereits ab 2,50 €, Monatsflats ab ca. 10 € angeboten.
Die Tarife gibt es sowohl mit Vertrag und entsprechender Laufzeit oder auch Prepaid mit Berechnung nach Aufwand. Bei den Prepaid Tarifen ist meist eine sofortige Kündigung möglich. Ein Vergleich lohnt sich.

4. Volumenbeschränkung
Datenvolumina gibt es von deutschen Anbietern momentan von 200 MB bis 5 GB. Nach Erreichen des Datenvolumens wird die Geschwindigkeit auf GPRS Geschwindigkeit gedrosselt, was für die Übertragung von medizinischen Bildern nicht ausreicht (max. 50 kBit/s).

5. Ausland
Die UMTS Nutzung im Ausland findet über das sog. Roaming statt. Hier sind aber die Gebühren so hoch, dass es sich meist lohnt eine UMTS SIM Karte im jeweiligen Land zu erwerben. Der Spiegel hat in dem Artikel "Die passende SIM-Karte für jedes Land" vom Mai 2011 Empfehlungen für 14 Länder abgegeben. Der Artikel erklärt welche Karten geeignet sind und wie man diese als Tourist erwerben kann.

Fazit
Mit dem iPad sind die Radiologen in der mobilen Welt angekommen. Auf Kongressen wie DRK, ECR, RSNA... ist die iPad-Dichte bereits sehr hoch. Der mobile Zugriff auf Patientendaten ist mit dem richtigen Tarif gut machbar.

iPhone Besitzer mit Telekom Vertrag können sich für einen einmaligen Bereitstellungspreis eine Multi SIM Karte zulegen, die dann ohne weiteren Gebühren im iPad verwendbar ist.*

Ansonsten ist der Flat Internet Optimum Tarif von Simyo (e-plus Netz) mit 1 GB Volumen pro Monat und 9,90 € monatlicher Gebühr für gelegentliche Teleradiologen zu empfehlen.*

Für Teleradiologen mit mehr Durchsatz (Datenvolumen bis 5 GB pro Monat) gibt es Tarife ab 14,95 € pro Monat.

Ausblick
In den nächsten Jahren wird der LTE Standard eingeführt, der Geschwindigkeiten von bis zu 50 MBit/s verspricht. Das sollte der mobilen Radiologie auf jeden Fall helfen.

Die OsiriX PRO Hands-On Workshops beim Deutschen Röntgenkongress waren ausgebucht. Mehr als 60 Kursteilnehmer lernten den Umgang mit OsiriX PRO für die Befundung und Nachverarbeitung von medizinischen Bilddaten.

Das Ergebnis kann sich sehen lassen. Mit einer Gesamtnote von 1,5 (nach Schulnotensystem) wurden die Erwartungen der Radiologen mehr als erfüllt.

An alle Kursteilnehmer nochmals vielen Dank für die Aufmerksamkeit und das konstruktive Feedback.

AGIT Workshop Open Source Software in der Radiologie beim Deutschen Röntgenkongress 2011

Im Rahmen eines @git Workshops der Deutschen Röntgengesellschaft anläßlich des 92. Deutschen Röntgenkongresses in Hamburg wurde am 1. Juni 2011 das Thema Open Source Software in der Radiologie diskutiert. Ca. 50 Teilnehmer bestehend aus Anwendern und Industrie interessierten sich für diesen Workshop.

Zwei Vorträge zu dem Thema aus Anwendersicht (PD Dr. Weisser, UMM) und aus Industriesicht (Stephan Popp, aycan) lieferten die Grundlage.

In vielen Kliniken und Praxen wird bereits Open Source Software (OsiriX, dcm4che, ClearCanvas,...) eingesetzt. Die Gründe hierfür sind geringe Kosten, einfache Lizenzierung und Verfügbarkeit und Funktionsvielfalt. Den Anwendern ist die Grenze zum Medizinprodukt und damit die regulatorischen Massnahmen nicht bewusst, bzw. wird dieser Aspekt verdrängt. Die Rolle als Hersteller (Stichwort: Eigenherstellung gem. MPG) nehmen viele Anwender nicht bewusst an. Die Universitätsmedizin Mannheim ist hier sicherlich die Ausnahme (siehe OpossUMM Projekt).

Der Aufbau eines QM-Systems konform der EU Direktive 93/42/EWG ist der Grundstein für die sog. Eigenproduktion. Eine Überblick über die Grundstruktur bietet nachfolgende Grafik.

Da die Einhaltung des Medizinproduktegesetzes und der Medizinproduktebetreiberverordnung aufwändig ist und hierzulande von keiner Behörde überprüft wird, ist das Risiko bei einem Off-Label-Use relativ klein. Dies wird von den Anwendern erkannt und auch bewusst in Kauf genommen. Die Risiken bezüglich Fehlbedienung, aufgrund von fehlender Dokumentation und Einweisung und die Fehlfunktionen bei den komplexen Softwaresystemen liegen beim Anwender.

Zusammenfassung

Open Source Software ist in der Medizin angekommen. Die regulatorischen Auflagen werden in der Regel nicht erfüllt. Der Nutzen einer zertifizierten Umgebung ist für die Mediziner nicht offensichtlich. Ein Betrieb als Medizinprodukt ist mit den notwendigen QM-Massnahmen möglich, wobei der Aufwand für ein adäquates QM-System sich nur schwer rechnet (siehe Radiologieforum Magazin 2-2011 Seite 10 "Open Source Software „Eigenherstellung“ in der Klinik legalnutzbar machen")

Great response from the 2011 German Radiology Congress

For the 16th time, aycan exhibited at the German Radiology Congress which was held from June 1 – 4, 2011.

The mobile classroom featuring hands-on OsiriX PRO workshops was well received by congress participants. One participant stated, "aycan brings a new dynamic to the radiology exhibition". Another visitor commented "A clear design of the booth and great hands-on workshops emphasize aycan’s competence". This is just a small sample of the positive feedback received from booth visitors.

aycan successfully tested its’ teleradiology app for use on the new iPad 2. The multi-threaded architecture of the aycan app harmonizes with the iPad 2’s new Dual Core 2 Processor. And, with the improved features and functionality of the iPad 2 over its predecessor, the aycan app works even better:

The transmission of images is clearly faster because the de-compression and decryption ability of the app is improved due to the iPad 2’s new A5 processor.

With the iPad 2’s new graphics’ chip, displaying images (scrolling and W/L) in split-view mode is significantly improved.

Heute haben wir das xr.ay App für die Teleradiologie auf dem neuen iPad 2 getestet.

Die Übertragung der Bilder ist deutlich schneller, da das dekomprimieren und entschlüsseln auf dem neuen A5 Prozessor beschleunigt wird. Die Multithread Architektur des xr.ay App harmoniert perfekt mit dem Dual Core iPad 2 Prozessor.

Beim Darstellen (Scrollen und Fenstern) im Splitview Modus bringt der neue Grafikchip eine deutliche Performancesteigerung.

Das Beste ist aber der Anschluß eines externen Displays an das iPad 2 mit der Full HD Auflösung von 1920x1080 Bildpunkten. Damit läßt sich die xr.ay App (Medizinprodukt Klasse I) an einen Befundmonitor anschließen und konform der DIN 6868-57 und der QS-RL betreiben.

OsiriX PRO 2.0 erhält FDA Clearance

Die Software ist bereits seit Ende 2010 als Medizinprodukt der Klasse IIb mit CE-LAbel in Europa erhältlich. Jetzt hat auch die US-amerikanische FDA die Marktzulassung im Rahmen einer 510(k) Premarket Notification als Class II Device erteilt.

Neben Bugfixes und Verbesserungen sind in der neuen Version einige Neuerungen implementiert.

ECR 2011 OsiriX PRO Workshop Summary

The workshops were listed in the official ECR 2011 program. 305 people registered at myesr.org prior to the congress.

Apple was very generous and sent 24 iMac's 27" to Vienna for the trainings.

400+ people attended the workshops. Each participant received a nice T-Shirt.

We performed 8 Basic Workshops and 8 Advanced Workshops with three different focuses, which was very well received by the workshop attendees.

The final statistics from ESR about the OsiriX PRO workshop at ECR 2011 are showing a better result than the 2010 results. 254 evaluation sheets were returned to ESR.

Featured in aycan's April 2011 issue of "News and Views"

aycan, with its' OsiriX PRO workstation, was among seven companies invited to demonstrate imaging systems at this year's European Society of Radiology Meeting.

aycan was among only seven companies invited to demonstrate their imaging system during the Workstation Face-Off session at this year’s European Society of Radiology (ESR) meeting. Imaging systems were selected that feature novel technological developments in diagnosis, therapy planning, and therapy guidance.

Coordinated by Prof. Hans-Christoph Becker and Dr. Anno Graser from Munich, this year's Face-Off addressed the industry’s challenge to meet the growing demand for more powerful image-data manipulation. A number of tasks were given to all participants such as providing 3D volume-rendered images of aortic valves from a superior perspective, along with maximum, minimum, and average diameter measurements. Another task was to define a target lesion, using RECIST (Response Evaluation Criteria in Solid Tumors) 1.1 Guidelines, and performing comparison assessments of lesions on more than two time-points. Using its OsiriX PRO (a cost effective, FDA and CE certified, 64-bit fast, highly-functional workstation for conventional, multi-slice and other image reading), aycan completed all the tasks successfully and in a timely manner.  

An ESR panel is currently assessing each company’s performance with findings to be released from the Society in the next few months. Final rankings will be based, in part, on the workstations ability to manage large volumes of data, and perform in a normal “reading room” setting as determined by expert users.   

Along with aycan, the other companies that participated were GE Healthcare, Philips Healthcare, Siemens Healthcare, Terarecon, Visage Imaging, and Vital Images.

Past participants have faced-off on other key areas including the measurement of stenosis in CT angiography, and perfusion imaging and analysis of CT and PET images. Emerging technologies demonstrated in past Face-Off sessions have led to advanced tool development including tools for more efficient readings such as 3D viewing, automated polyp detection, and efficient length and volume measurements. This year, new computer aided diagnosis (detection) techniques demonstrated could be instrumental in the development of a more efficient and clinically relevant workflow.

How to Choose

Amidst ever-changing technology and the ongoing debate on standards and regulations for medical monitors, how do you choose one that will support increased productivity and more accurate readings?

With numerous monitors available on the market, varying needs of users, and ever-increasing budget pressure, its important to select the right monitor for your needs. Below is a basic overview of how to go about selecting a monitor along with detailed guidelines from the industry’s leading bodies.

First, Determine the Application

Choosing a medical monitor depends first on what you plan to use it for. Outside of general office monitors, there are two categories of monitors in the digital medical environment: Clinical and Diagnostic.

Clinical Monitors, used for displaying clinical records and DICOM images, are a cost efficient option to a full-fledged diagnostic monitor, with many of them delivering calibration that is compliant with DICOM Part 14.

Diagnostic Monitors have higher performance values than Clinical Monitors with brightness and grayscale being especially important as they provide greater image detail which is essential for readings.

Consider Key Performance Factors

No matter which category you fall in, determining a monitor’s performance for productivity and accuracy is based on the same key factors:

Contrast. Contrast is simply the ratio of the luminosity of the brightest color (white) to that of the darkest color (black) that the monitor is capable of producing. The greater the contrast, the greater the detail.

Dependability. A monitor should be stable so that what a user sees today is the same as what they’ll see tomorrow. FDA clearance and warranties are two items that indicate a monitor’s dependability.

Grayscale. For black and white images, being able to render every shade of gray as defined by Digital Imaging and Communications in Medicine (DICOM), the American College of Radiology (ACR), and the American Association of Physicists in Medicine (AAPM) guidelines, is essential for accurate readings. The greater the grayscale, the more detail you will see in the image, which is particularly important when viewing mammography and CR images. A DICOM grayscale look-up table is also important so that grayscale output is optimized to the human visual system.

Luminance. Luminance is used to characterize the brightness of displays. A typical computer display emits between 50 and 300 cd/m2. For diagnostic displays, between 400 and 900 is recommended. In general, the greater the luminance the better.

Resolution. Resolution refers to the pixel density—the number of pixels per-unit distance or area. As a general rule, the higher the resolution the sharper the image will appear. This means less clicking, panning and zooming a user will need to do to see what they need to see.

Uniformity. An image should look the same on any monitor connected to the same network. This is particularly critical when a user views images on multiple monitors, or when sharing or collaboration is needed. Uniformity depends on the quality of the monitor's software and hardware.

Viewing Angle. Viewing Angle is the maximum angle at which images on the monitor can be viewed without excessive degradation to the image. It is measured in degrees horizontally and vertically. The average viewing angle found in medical monitors is 170 degrees. And again, as a general rule, more is better. The greater the angle, the better the view.

Understand Legal Requirements and Standards

While there are no legal requirements on which medical monitor to use, the Federal Drug Administration (FDA), the ACR, and the AAPM have all created guidelines for the electronic viewing of medical images. These guidelines cover the performance factors discussed above as well as test patterns for sharpness at the center of the four corners, visibility of rectangles, and tests to determine if the display is free of shadows and image movements. Uniformity test criteria for luminance, and grayscale are also provided. All toll, the guidelines set best practices for creating an optimal diagnostic reading environment. NEMA: http://medical.nema.org/dicom/2004/04_14PU.PDF This guideline, DICOM Part 14: Grayscale Standard Display Function, from the National Electrical Manufacturers Association (NEMA) specifies a standardized Display Function for display of grayscale images. Both the ACR and AAPM standards recommend the use of DICOM Part 14.

FDA: http://www.fda.gov/MedicalDevices/DeviceRegulationandGuidance/default.htm Although not required, for quality and liability reasons, it’s recommended to go with an FDA-cleared monitor.

Consider Cost

Of course, cost is also a key factor. Because the price difference between the three types of monitors is significant, it’s important to choose the right one for your application. On average, good quality General Office Monitors range in price from $200 to $500; Clinical Monitors range from $1,500 to $5,000, and Diagnostic Monitors price from $6,000 on up. The important thing is not to over buy or under buy for the intended application.

Watch Technology Trends

As technology continues to advance at a rapid pace, its important to understand the trends so you acquire a monitor that will give you the performance you need today and well into the future. Some trends to consider when selecting a monitor are:

DisplayPort. DisplayPort is the new standard for connecting monitors to graphic cards--replacing the Digital Video Interface (DVI) for this purpose. It’s a small, easy-to-use connector with a simple locking mechanism that eliminates the need for screws. DisplayPort works with longer cable lengths and has potential for daisy chaining monitors (an interconnection of monitors, one after another) and other advanced features. DisplayPort can provide the same functionality as a High-Definition Multimedia Interface (HDMI) but is not expected to displace HDMI in high-definition consumer electronics devices. Most new graphics cards have DisplayPort connectors.

Eco-Friendly Monitors. Combining ergonomic and environmental features, Eco-Friendly Monitors deliver increased comfort for improved productivity, reduced environmental burden, and ultimately lower running costs.

3D Stereoscopic Imaging. This technique creates/enhances the illusion of depth in an image in one of three ways: by viewing separate images from two offset sources using glasses; using glasses that filter offset images from a single source separated to each eye; or viewing without glasses as the light source splits the images directionally into the viewer's eyes.

Quad HD Monitors. Quad HD is the next step in high-resolution large screen monitors. Instead of using many small monitors, interventional suites and operating rooms can now use one Quad HD Monitor that can be configured and optimized for image presentation from multiple inputs and uniquely configured for best-image presentation by doctor preference or procedure.

OLEDs, E ink, and Portables (iPads, iPods, Tablets). OLED and E ink are new display technologies that could replace the LCD display. E ink is used in the Kindle, for example, and has superb viewing characteristics under daylight conditions. OLED is currently being used in some phones and small displays. Known for vibrant color reproduction and a slim form factor, OLEDs are still very expensive and there are concerns about degradation over time. iPads and iPods are appreciated for their portability, superb image quality, and providing instant access to information anytime, anywhere. Of course they have some drawbacks when compared to the criteria established by ACR and AAPM.

Look at Maintenance and Warranty

Once you’ve chosen the right monitor for the job, you’ll want to be sure it is maintained for optimal performance. This means ongoing monitoring and calibration. To minimize time and expense and ensure consistency, Quality Assurance (QA) software is available from many monitor vendors that can automate QA tests such as calibration as well as provide evidence of compliance with laws.

To protect your investment, it’s best to obtain a warranty that provides a minimum of three to five years of comprehensive coverage. The warranty should also guarantee the total image quality including brightness and color temperature.

Summary

While reading-environment conditions and a variety of software and hardware components affect image quality and interpretation, the most influential component is the monitor. To ensure you choose the right one, first determine the use of the monitor, either Clinical or Diagnostic, and then select one that aligns with the guidelines set by the industry’s key governing bodies. Be sure to also review and consider both warranty and maintenance plans as well as future technology trends as both of these can present added expense if not factored into your initial purchasing decision.

The Latest on iPads, Apps, and the FDA

Since its inception in 1976, Apple technology has revolutionized many industries including graphic arts and printing, music, and mobile connectivity. Today, Apple technology, along with third party solutions, is transforming the medical industry.

The impact of Apple in medicine began with use of its development tools. As an example, in 2004, Apple’s well-conceived integrated development tools led to the most successful Open Source project in medicine to date, OsiriX. Developed by a team from the University of Geneva, under the lead of Professor Dr. Osman Ratib and Dr. Antoine Rosset, both trained Radiologists, OsiriX is an image-processing application dedicated to DICOM images.

After the Open Source was introduced, commercial entities began to offer various value-adds including plug-ins for radiology applications such as hanging protocols, AAA measurement tools, lung-nodule detection algorithms, and an ejection fraction tool. Medical certification, such as FDA and CE Mark, also became available for OsiriX and many of its applications. Other value-adds from commercial suppliers include support, training, and integration services. Open Source and commercial versions of OsiriX are now running on more than 60,000 Apple desktops/laptops and continue to grow in acceptance and use. Outside of its intrinsic value as a tool, OsiriX has allowed the medical community unprecedented access to a high-end DICOM viewer.  

While software created with Apple’s development tools has impacted radiology and a limited number of other medical communities, it wasn’t until the introduction of the iPad in 2010 that the entire medical community was affected by Apple technology. From administration, to clinical work, to surgery, the iPad is increasingly being used in all areas of medicine due to its extreme portability, superb image quality, large screen size, and the multitude of medical apps being developed to run on it.  

The first FDA approved medical app came in February 2011 with Mobile MIM from MIM Software. Approved for review and medical diagnoses of MRI, CT and PET scans, this breakthrough paved the way for future apps to be approved. And not long after, the FDA did just that with approval of Mobisante’s MobiUS for smartphone-based ultrasounds. Other hot apps currently being reviewed are for preliminary interpretation of brain CT scans, and for evaluating for the presence or absence of tuberculosis. The app-development trend is expected to continue as demand from the medical community continues to increase for solutions that increase productivity and drive down costs.   

In addition to apps, the portability of the iPad is changing how medical professionals work. For example, the University of Chicago issued over 100 iPads to residents in their medical center. According to a report from NBC, the iPad is allowing residents instant access to patients’ electronic medical records, and they in turn are able to share charts and medical images with patients at their bedsides. This allows patients to participate more in their own care and allows residents the ability to provide minute-to-minute care. This workflow improvement has also allowed residents to spend more time with their patients and less time filling out paper work—in some cases paperwork has reportedly been cut in half.   

The portability of the iPad is also contributing to growth in Teleradiology, the practice of remote review, interpretation, and diagnosis of radiological images. The iPad allows radiologists to work more easily from remote locations, which is particularly important for patients in rural areas where specialists are not available. As more apps and mobile versions of PACS systems emerge for the iPad, Teleradiology will continue to grow and become more effective.

  Release of the iPad 2 (March 2011) brings even more potential for its use in medicine given its improved processor and operating system, lighter weight, and smaller size. All these advancements improve ease of mobility and performance.

  In addition to Apple development tools and iPads, there are other Apple products and apps that have use in medicine. And keeping pace with all this potential is the FDA. Due to increased marketing, interest, and use, the FDA is paying closer attention to Apple products and apps for the medical community. In particular, they have expressed much concern about mobile technologies being used for diagnostic purposes. With Apple medical solutions now solidly on the FDA’s radar, it will be even more challenging for mobile-technology application providers to obtain FDA clearance and approvals.  

As Apple technology continues to evolve, and companies continue to develop and obtain FDA approval of medical apps, we anticipate the Apple in Medicine List to grow along with greater productivity for its users and better care for patients.

Kongress für Technologien in der Medizin und Energieeffizienz in Kliniken vom 23.-25.5.11 in Würzburg

Die Tagung findet zum 12. Mal in Würzburg statt und ist die wichtigste deutschsprachige Konferenz für angewandte Technik in der Medizin Europas.

Teilnehmer sind Führungskräfte aus der Gesundheits- und Energiewirtschaft: Geschäftsführung, Entwicklung, Leiter der Bereiche Medizintechnik, IT, Energie, Gebäudetechnik und Bau, Hygiene sowie Einkauf.

aycan releases another clinical application plugin for OsiriX PRO. The AYEjectionFraction Plugin supports the evaluation of coronary CTA.

The AYEjectionFraction Plugin enables the user to calculate the left ventricle ejection fraction (LV EF) with the Dodge correction formula.

The AYRecist plugin is available at the end of March 2011 in Europe as option for the CE-labeled OsiriX PRO.

aycan releases another clinical application plugin for OsiriX PRO. The AYRecist Plugin is for the evaluation of oncological followups according to the RECIST 1.1 guidelines.

The AYRecist Plugin supports the user with an unlimited number of followups. After definition of the Target Lesions and Non-Target Lesions by the user it will display baseline and followup lesion synchronized. The AYRecist Plugin also shows a table of the RECIST parameters and the response over time. The key images and the table can be embeded into a report and exported as DICOM files to the PACS.

The AYRecist plugin is available at the end of March 2011 in Europe as option for the CE-labeled OsiriX PRO.

The software was presented during the 4th ECR Workstation Face-off at ECR 2011 in Vienna.

About RECIST:
RECIST (Response Evaluation Criteria In Solid Tumors) is a set of published rules that define when cancer patients improve ("respond"), stay the same ("stable") or worsen ("progression") during treatments.
The original criteria were published in February 2000 by an international collaboration including the European Organization for Research and Treatment of Cancer (EORTC), National Cancer Institute (NCI) of the United States and the National Cancer Institute of Canada Clinical Trials Group.
RECIST 1.1, published in January 2009, is an update to the original criteria. Today, the majority of clinical trials evaluating cancer treatments for objective response in solid tumors are using RECIST.

Auf der RSNA 2010 zeigen wir neben einer neuen OsiriX PRO Version erstmals zwei neue OsiriX PRO Plugins.

Das erste Plugin ermöglicht die Abnahme- und Konstanzprüfung von medizinischen Bildwiedergabegeräten nach DIN 6868-57 (Sicherung der Bildqualität in röntgendiagnostischen Betrieben - Teil 57: Abnahmeprüfung an Bildwiedergabegeräten) und QS-RL.

Das zweite Plugin unterstützt die die Abnahme- und Konstanzprüfung nach DIN 6868-159 (Sicherung der Bildqualität in röntgendiagnostischen Betrieben - Teil 159: Abnahme- und Konstanzprüfung in der Teleradiologie nach RöV).

Das 96. Jahrestreffen der Radiological Society of North America (RSNA) findet auch in diesem Jahr wieder in Chicago statt und steht unter dem Motto "Personalized Medicine". Das Treffen ist weltweit das Wichtigste und Größte seiner Art. Vom 28. November bis zum 3. Dezember werden ca. 50.000 Teilnehmer in der Windy City erwartet. Neben den zahlreichen wissenschaftlichen Präsentationen und Fortbildungen wird die angegliederte Industrieausstellung das Interesse der Besucher auf sich ziehen. In drei Hallen präsentieren die Aussteller ihre Produkte und Lösungen.

Heute hatten wir bei aycan Gelegenheit den 12 Core Mac Pro mit OsiriX PRO zu testen. Das System beeindruckt zuerst einmal mit der rasanten Ladegeschwindigkeit. 1600 CT Bilder werden in 8 Sekunden von der Festplatte (10.000 U/min) in den 32 GB großen Arbeitspeicher geladen. Dies entspricht einer Ladegeschwindigkeit von 200 CT-Bildern/Sekunde.

Das ressourcenhungrige 3D Volumerendering läuft mit der Becken-Bein CTA-Serie mit 1600 Bildern ruckelfrei in höchster Qualität auf einem 5 Megapixel Monitor. Die 12 Prozessorkerne werden beim rotieren maximal zu 2/3 ausgelastet.

Fazit: Der über 12 Proressorkerne verfügende Mac Pro ist im Zusammenspiel mit OsiriX PRO eine sehr schnelle DICOM PACS Workstation, die in punkto Leistung, Bedienbarkeit und Funktionsvielfalt ihresgleichen sucht.

Die Partnerschaft ermöglicht Ärzten das von SOVAmed entwickelte Plugin SOVA.evar in OsiriX PRO - der CE-gelabelten und FDA-freigegebenen Version des erfolgreichen Open Source DICOM Viewers OsiriX - zu nutzen.

Das Plugin ist eine medizinische Applikation zur Visualisierung und Vermessung von abdominalen Aortenaneurysmen (AAA). Es handelt sich um ein Plug-in für OsiriX PRO, eine hochqualitative Software für Apple Mac Computer.

Auf dem 26. Jahrestreffen der Deutschen Gesellschaft für Gefäßchirurgie vom 7.-11. September 2010 wird das Plugin für die Morphometrie von Aortenaneurysmen erstmalig dem Fachpublikum vorgestellt.

Das Gesamt-Softwaresystem wird in Q4/2010 als zertifiziertes Medizinprodukt erhältlich sein. DIN 6868-57 Sicherung der Bildqualität in röntgendiagnostischen Betrieben - Teil 57: Abnahmeprüfung an Bildwiedergabegeräten

Seit Februar 2001 regelt die DIN 6868-57 die Abnahmeprüfung an medizinischen Bildwiedergabegeräten.

Diese Norm gilt für die Darstellung von SW-Bildern auf Bildwiedergabegeräten (BWG) in Form von Grauwerten auf Bildschirmen. Sie beschreibt die Durchführung von Abnahmeprüfungen zur Feststellung der Bildqualität von BWG.

MedConf 2010: Software- und Systementwurf für Medical Devices

Die MedConf hat sich in den letzten beiden Jahren als wichtiger Kongress im Umfeld der Medizintechnik etabliert. Sie adressiert brisante Themen wie das neue MPG (Medizinproduktgesetz), Auswirkungen gesetzlicher Rahmenbedingungen durch die EU oder die FDA sowie Methoden und Techniken des Software Engineerings innerhalb der Entwicklung. Namhafte Unternehmen - wie aycan - stellen ihre gesammelten Erfahrungen in Vorträgen und Workshops dar. Neben dem neuen MPG stellt das Thema Usability medizinischer Software einen weiteren Themenschwerpunkt dar.

Die MedConf selbst gliedert sich in einen Workshoptag (12. Oktober) sowie die beiden Kongresstage (13. bis 14. Oktober 2010). Am Workshoptag werden ein Halbtages- und vier Ganztagesworkshops angeboten, in denen die Konferenzthemen vertieft werden. An den beiden Konferenztagen finden die Teilnehmer einen Mix aus Anwenderberichten von Medizintechnikunternehmen wie aycan, Fachvorträgen von Dienstleistungsunternehmen, deren Portfolio sich auf die Medizintechnik konzentriert und Methodenvorträgen von Consultinghäusern, deren Erfahrungen aus ebenfalls regulierten Branchen sich auf die Medizintechnik übertragen lässt.

aycan ist diese Jahr auch wieder mit einem Vortrag "Open Source Software als neues Paradigma für Medizinprodukte" am 14.10.2010 am Start.

aycan installierte zusammen mit dem mexikanischen Vertriebspartner Medinuvo S.A., erfolgreich das erste aycan xray-print System im Universidad Autónoma de Nuevo León hospital in Monterey, Mexiko.

The Universitätsklinikum ist ein 500 Betten Haus und ist eines der wenigen Lehrkrankenhäuder in Mexico.
